Nicole spent two years doing everything she thought she was supposed to do to feel better.

She exercised regularly. She cleaned up her diet. She prioritized sleep. She made time to take care of herself.

Yet the number on the scale wouldn't budge - and the exhaustion wasn't getting any better.

She could sleep for nine hours and still wake up feeling like she'd run a marathon.

Why Can Unexplained Weight Gain and Fatigue Happen Together?

Fatigue and weight gain are symptoms—not diagnoses.

There are many potential contributors, including sleep disruption, metabolic health, thyroid issues, gut dysfunction, nutritional deficiencies, medications, stress, changes in muscle mass and activity, and the hormonal changes that occur during perimenopause and menopause.

That's why I don't automatically assume that unexplained weight gain means “your hormones are off” or that fatigue means “your cortisol is broken.”

Symptoms give us clues.

Then we investigate.

For Nicole, the important question wasn't simply:

How do we get the weight off?

It was:

Why has your body stopped responding the way it used to?

That's a very different conversation.

The Investigation: Testing + Active Listening

When Nicole came to me, I didn't start with a supplement protocol.

I started with her story.

What was she experiencing? When did it begin? What had changed? What had she already tried? What testing had already been done? What did her lifestyle look like? What were her goals?

This is an important part of the work I do. 

I don't believe in ordering every functional test available simply because we can.

Test, don't guess doesn't mean test everything.

It means understanding the person first and then choosing the testing that may help answer the questions raised by her history and symptoms.

Based on Nicole's history, I recommended targeted functional testing.

Her results revealed findings involving her gut and hormone patterns that gave us additional information to consider alongside what she was experiencing. The pieces were beginning to make more sense.

From Test Results to a Personalized Plan

Getting test results isn't the finish line.

Knowing what to do with the information is what matters.

Once Nicole's results came back, I developed a personalized plan based on her findings, symptoms, lifestyle and goals.

Her recommendations weren't pulled from a generic “weight-loss protocol.”

That's an important distinction.

Why Trying Harder Wasn't the Answer

So telling her to simply eat less and exercise more wasn't particularly useful.

Sometimes the next step isn't trying harder.

This is where I believe the “health detective” approach becomes so valuable.

I'm not looking at weight in one box, fatigue in another, digestion somewhere else and hormone symptoms as an entirely separate problem.

I'm asking whether those symptoms may be giving us clues about a bigger picture.

What Should You Investigate If You're Gaining Weight and Always Tired?

The first step is resisting the temptation to diagnose yourself from one symptom—or one TikTok video.

Fatigue plus weight gain doesn't automatically mean hypothyroidism, menopause, insulin resistance, high cortisol or a gut problem.

Instead, I want to understand the complete picture.

Questions I might consider include:

  • When did the symptoms begin, and what changed around that time?
  • How well are you actually sleeping?
  • Have your menstrual cycles or other hormone-related symptoms changed?
  • What medications or supplements are you taking?
  • What has already been tested?
  • Are you experiencing digestive symptoms?
  • Have your hunger, cravings or appetite changed?
  • What does your metabolic health look like?
  • Could nutritional deficiencies be contributing?
  • How much stress is your body managing?
  • Are you maintaining adequate muscle mass and physical activity?

The answers help determine what deserves a closer look.

And sometimes that means functional testing.

Other times, conventional bloodwork or further evaluation with a physician is the more appropriate next step.

The goal isn't more testing. The goal is better information.

Information Changes the Conversation

What I provided Nicole wasn't a magic solution.

It was investigation, information, strategy, advocacy and a plan built around her.

That's what I believe personalized functional health should look like.

I'm not satisfied with automatically dismissing a woman's symptoms as “just getting older.”

But I'm equally unwilling to blame every symptom on hormones, cortisol, toxins, parasites or whatever happens to be trending on social media this week.

Your symptoms are information.

Our job is to figure out what they're telling us.
